I would like to purchase a men's ring, wedding band or similar and have a Ruby placed in it. I am not sure if jewelers will do this, and if so will they do it for a ring purchased elsewhere? Or, must it be done to a ring that already had a setting, or can it be added to a smooth, plain ring? It's very difficult to find mens rings with gemstones- any advice would be helpful!
Yes, you have a few different options here...
1) Places like Zales have monthly events, where a jeweler comes in and you can choose a setting and a stone and mix & match the way you want.
2) Check out the jeweler's catalog Rio Grande at http://www.riogrande.com, and you can buy the prong setting separately from the band, and then have any reputable jeweler put them together. It's actually a really simple thing to do. You can also buy empty ring settings.
3) You can buy your gem at an online website like Thai gems, http://www.thaigem.com or on eBay, etc., and take it to your local jeweler.
4) A good, local jeweler can help you at any stage in this project.

Engagement ring has a ruby in it and has been passed down through generations. We are wondering what type of wedding band/ring you get to accompany the gemstone ring and on which finger do you wear it? Same finger as the Eng. Ring or on the other hand?
There is no set rule here. If the wedding ring or band fits well with the engagement ring then you can wear the rings together. She can wear the engagement ring on her right had, too. I suggest talking to your fiance's mother or grandmother and find out how this question is traditionally handled in her family...

There is no correct answer for all types of Gemstones.
First I should not that want to be very careful with such gemstones as Opals and Turquoise, and such!
